We offer DOT Medical Exams for both individual drivers and employers needing group scheduling. Individual exams are $100 with on site visits available for groups of 5 or more drivers scheduled together in Niagara County. *Travel fee applies for on site visits in surrounding counties.

A DOT medical exam (DOT physical) evaluates your overall health to determine if you meet FMCSA medical standards for commercial driving.
During your exam, we assess:
- Vision and hearing
- Blood pressure and heart health
- Neurologic and musculoskeletal function
- Respiratory health
- General medical history
- Urinalysis (required by FMCSA for medical screening)

Your exam will be completed by a FMCSA-certified Medical Examiner, and results will be submitted directly to the FMCSA National Registry as required.
If you qualify, you will receive your DOT Medical Examiner’s Certificate at the visit.

- List of current medications
- Medical records or clearance letters (if you have conditions such as high blood pressure, diabetes, sleep apnea, heart disease, or seizures)

DOT Medical Exams are available by appointment only.
Appointments can be scheduled online now through our booking system.
- Please arrive 10–15 minutes early to complete required paperwork
- Most DOT exams take around 30 minutes
If you have a medical condition such as high blood pressure, diabetes, sleep apnea, heart disease, or a neurologic condition, please bring any relevant medical records or clearance letters to your appointment to avoid delays.

- Same-day Medical Examiner’s Certificate if qualified

Payment is due at the time of service.
Please note:
- Certification outcome does not affect exam fee
- Additional visits or clearance reviews may require an additional fee
- DOT exams are not covered by insurance

We offer group DOT Medical Exams for employers, fleets, and organizations with multiple drivers needing certification.
- Dedicated group appointment blocks at our clinic in Newfane, NY available
- On-site group exams are available by request
- On-site group exams performed at a rate of $100 per exam throughout WNY
- On-site exams provided in Niagara County, NY have no additional travel fees. A travel fee will apply to surrounding WNY counties.
